KINDERGARTEN SCREENING
Parents will meet with the school nurse to collect health information for students entering Kindergarten in the fall of 2012.
Please be sure to bring your child;s
most recent physical with documantation of vision screening,
all immunizations,
and
completed parent questionnaire
You will meet with the school nurse for a short period of time to review these documents.
Illness
If your child has been ill and is returning to school on medication
If your child is ill and you are unsure of when he or she should attend school,
Vision Screening Guidelines
If your child wears glasses, please be sure that they have them here at school to ensure a successful school day.
Massachusetts State Law requires students to have vision screening at school.
In Salem we screen Pre K through grade 5, grades 7 and 10.
Under the new guidelines, children will now be screened in both near and distant vision.
Pre K through grade 3 will also have steriopsis screening.
Parents will be notified in writing if their child fails the screening.
The form used can be given to an eye doctor for further screening evaluation and then returned to the school nurse.
The new guidelines were developed by the Department of Public Health with a physician to ensure better screening of the children.
Current Immunizations
All immunizations must be up to date for a child to enter school.
Medication During School Hours
If your child needs medication during school hours
(medication forms are available to download on this web site on the elementary, middle and high school pages)
BMI ---- Body Mass Index
Students in grades 1. 4. 7. and 10
will have yearly heights and weights done and their BMI's calculated by the school nurses.
Parents will be notified of any student with a BMI in the high risk category.BMI.htm
